Account, Profile, Activity, and Use InformationWe collect basic account information such as your name, email address, date of birth, gender, username and password that helps secure and provide you with access to our Services.
Profile, activity and use information is collected about you when you turn Start Monitoring on or choose to upload a picture, activity or otherwise use the Services.
We use your contact information so we can respond to your support requests and comments.
Location InformationWe collect and process location information when you sign up for certain subscription tiers and use the Services. We do not track your device location while you are not using Smart Monitor. Location data will be sent as part of a text message, phone call, email or any combination of these to provide services for the subscription tier and will be stored as part of your data on our secure cloud and can be accessed by you on our secure user portal.
Contact InformationSmart Monitor provides a fee-based service (at our sole discretion) in which Smart Monitor attempts to initiate contact with the contacts you have listed on the app via phone call, text message, email or any combination of these based on user settings.
Connected Devices and AppsSmart Monitor collects information from devices and apps you connect to Smart Monitor Applications. For example, you may connect your Apple watch or Samsung smartwatch Smart Monitor and information from these devices and apps will be accessed by Smart Monitor Applications.
Health InformationSmart Monitor may collect or infer health information. Certain health information such as heart rate, motion, other information such as videos and/or images may be inferred from various sources such as accelerometer, gyroscope, heart rate sensors, audio and video sensors. Before you start using the Services, you will be asked to provide consent to the collection, use and storage of such information. You may choose to use certain features of our Application that will allow you to input information with respect to your health such as symptoms, share images and/or videos, allergies and information about the medications you take such as name, dosage etc.
Technical Information and Log FilesWe collect information from your browser, computer, or mobile device, which provide us with technical information when you access or use the Services. This technical information includes device and network information, geolocation information (to use as part of text message, email and/or phone call notifications) log files and analytics information.
The Services use log files. The information stored in those files includes IP addresses, browser type, internet service provider (ISP), referring/exit pages, platform type, date/time stamp, and number of clicks. This information is used to analyze trends, administer, protect and secure the Services, track member movement in the aggregate, and gather broad demographic information for aggregate use. Smart Monitor engages providers that are industry leaders in online security, including Services verification, to strengthen the security of our Services. The Services are registered with site identification authorities so that your browser can confirm Smart Monitor’s identity before any personal information is sent. In addition, Smart Monitor’s secure servers protect this information using advanced firewall technology.
